Introdução: A obesidade é considerada uma pandemia global e fator de risco para o desenvolvimento de doenças cardiovasculares (DCVs). Evidências recentes sugerem que a pressão arterial central (PAc) está fortemente associada com as DCVs. O aumento da rigidez arterial (RA) é o principal fator associado ao aumento da PAc. O treinamento aeróbio é capaz de promover a redução da PAc. Entretanto, pouco se sabe sobre o impacto de diferentes modalidades de treinamento aeróbio sobre a PAc. O treinamento intervalado de alta intensidade (HIIT) tem sido considerado uma alternativa ao treinamento contínuo de intensidade moderada (MICT) tendo em vista sua superioridade na melhora da aptidão cardiorrespiratória e de outros parâmetros de saúde. Contudo, a superioridade do HIIT na melhora da PAc ainda precisa ser elucidada. Portanto, os objetivos da presente dissertação foram: 1. Revisar os efeitos crônicos do treinamento intervalado de alta intensidade (HIIT) versus o treinamento contínuo de intensidade moderada (MICT) sobre a PAc; 2. Investigar os efeitos do HIIT versus MICT sobre a PAc e rigidez arterial em mulheres obesas jovens. Métodos: A revisão sistemática e meta-análise foi conduzida conforme o PRISMA e registrado no PROSPERO (CRD42018111573). A busca dos ensaios clínicos randomizados foi realizada em cinco bases de dados eletrônicas (Pubmed/Medline, Web of Science, Cochrane, Lilacs e Scielo). Foi pesquisado estudos que compararam os efeitos crônicos do HIIT versus o MICT sobre a PAc, também foi extraído destes estudos os valores para a PAp, RA e VO2máx. A meta-análise foi conduzida no software Review Manager® e os dados foram apresentados como diferença média padronizada (SMD), com seus respectivos intervalos de confiança de 95%. Utilizamos o modelo de efeitos aleatórios para reduzir a influência da heterogeneidade e comparar as alterações da PAc, PAp, RA e VO2máx após HIIT e MICT. Para o estudo experimental de design paralelo, participaram 25 mulheres jovens com obesidade grau I e II. Foram avaliadas as medidas antropométricas, composição corporal e aptidão cardiorrespiratória. As principais medidas de rigidez arterial mensuradas foram a velocidade da onda de pulso (VOP) carótida-femoral, Augmentation Index (AIx), Aix@75 e também a pressão arterial periférica e central, sendo, sistólica, diastólica e de pressão de pulso pelo equipamento SphygmoCor®. As avaliações foram realizadas antes e após o período de treinamento de 8 semanas. As voluntárias foram alocadas aleatoriamente em dois grupos: HIIT (4 estímulos de 4 minutos de exercício na intensidade entre 85 e 95% da FCmáx, alternados por períodos de 3 minutos de recuperação ativa na intensidade entre 65 e 75% da FCmáx) ou MICT (41 minutos na intensidade entre 65 e 75% da FCmáx). Para a comparação entre os grupos (HIIT e MICT) e entre os momentos de avaliação (pré e pós) foi utilizada a ANOVA para medidas repetidas, seguido da correção de Bonferroni. Adotou-se o valor de p<0,05 para significância estatística. Resultados: Inicialmente, 1409 artigos foram identificados para compor a revisão sistemática com meta-análise. Após aplicação dos critérios de elegibilidade, sete artigos envolvendo 196 participantes foram incluídos na meta-análise. Ao comparar as mudanças pré e pós HIIT e MICT, não houve diferença significantativa para a pressão arterial sistólica central (PASc) (SMD -0,13 [IC95%, -0,47 a 0,21], p = 0,45, I² = 25%) e para a PADc (SMD 0,15 [IC95%, -0,15 a 0,46], p = 0,32, I² = 20%). Quanto as variáveis secundárias, não houve diferença significante para a pressão arterial sistólica periférica (PASp) (SMD -0,23 [IC95%, -0,52 a 0,06], p = 0,12, I² = 10%), pressão arterial diastólica periférica (PADp) (SMD -0,02 [IC95%, -0,30 a 0,27], p = 0,91, I² = 11%), velocidade de onda de pulso (SMD 0,18 [IC95%, -0,12 a 0,48], p = 0,24, I² = 4%) e augmentation index (AIX)(SMD -0,27 [IC95%, -0,75 a 0,21], p = 0,27, I² = 46%). No estudo experimental, tanto o HIIT como o MICT reduziram significativamente a VOP (Δ = -0,37 ± 0,2, p <0,001 e Δ = -0,35 ± 0,3, p <0,001, respectivamente). Além disso, houve redução significante na PAc sistólica (Δ = -6,6 ± 6,8, p = 0,010, no augmentation pressure (AP) (Δ = -3,0 ± 5,1, p = 0,033) e tendência à redução no AIx (Δ = -6,4 ± 14,8, p = 0,092) e AIx @ 75 (Δ = -7,6 ± 15,1, p = 0,053) após o HIIT e da PAc diastólica (Δ = - 3,6 ± 5,0, p = 0,026) após o MICT. Conclusão: Com base nos achados da revisão sistemática com meta-análise, o HIIT e MICT foram similares na redução da PAc, PAp e RA na população saudável ou em condição especial de saúde. Dessa forma, ambos os TA podem ser utilizados na melhora dos parâmetros hemodinâmicos centrais e na saúde arterial. De forma similar, no estudo experimental, ambos os protocolos HIIT e MICT reduziram a RA em mulheres jovens obesas. Contudo, apenas o HIIT foi capaz de reduzir a PAc e a amplificação da pressão, sugerindo que o HIIT poderia ser superior ao MICT na melhora dos parâmetros hemodinâmicos centrais nessa população.
Abstract:
Introduction: The obesity is considered a global pandemic being also a risk fator for the development of cardiovascular diseases (CVDs). Evidence suggests that the central blood pressure (cBP) is strongly associated with the CVDs. The increase in the arterial stiffness (AS) is the main factor associated with an increase in the cBP. The aerobic training is capable to promote the reduction of cBP. However, little is known about the impact of different aerobic training modalities on the cBP. The high-intensity interval training (HIIT) has been currently considered as an alternative to the moderate-intensity continuous training (MICT), mostly by virtue of its superiority in enhancing cardiorespiratory fitness and other health-related parameters. Nevertheless, the superiority of HIIT in promoting greater improvements on cBP still needs to be further explored. Thus, the aims of the present master’s degree thesis were: 1. To revise the chronic effects of HIIT versus MICT on cBP; 2. To investigate the effects of HIIT versus MICT on cBP and AS in young obese women. Methods: The systematic review and meta-analysis was carried out according to PRISMA guideline and previously registered on PROSPERO (CRD42018111573). Search of randomized controlled trials occurred in five different electronic databases (Pubmed/Medline, Web of Science, Cochrane, Lilacs and Scielo). Studies which compared the chronic effects of HIIT versus MICT on central blood pressure (cBP) were considered for inclusion. Data regarding peripheral blood pressure (pBP), arterial stiffness (AS) and maximal oxygen uptake (VO2max) were also extracted from the included studies. The meta-analysis was performed with the aid of the software Review Manager® and data was presented as standard mean deviation (SMD), with a confidence interval of 95%. We used the random effect model in order to reduce the heterogeneity and also compare the changes on cBP, pBP, AS and VO2max after HIIT and MICT. Regarding the experimental study design, 25 young obese women (body mass index between 30 and 39.9 kg/m2) participated in the study. Anthropometric, body composition and cardiorespiratory measurements were collected. The main variables of AS were the carotid-femoral pulse wave velocity (PWV), augmentation index (AIx), AIx adjusted for 75 bpm (AIx@75) and also central e peripheral blood pressures as well as pulse pressure. The hemodynamic assessments were performed by the SphygmoCor®. The evaluations were performed before and after the 8-week training period. Volunteers were randomly allocated in two groups: HIIT (four exercise bouts of four minutes each with the intensity ranging from 85% and 95% of maxHR, interspersed by 3-minute periods of active recovery with the intensity between 65% and 75% of maxHR), or MICT (41 minutes between 65% and 75% of maxHR). Data concerning the comparison between groups (HIIT vs. MICT) and between moments (pre and post intervention) were analysed using the ANOVA for repeated measures, followed by the Bonferroni correction. A significance level of P<0.05 was adopted. Results: Primarily, after the key terms were uniformly applied in all electronic databases,1409 references were found for screening. After references screening according to the previous established eligibility criteria, seven references, including altogether 196 individuals, were included in the present systematic review and meta-
analysis. After comparing the baseline and post intervention changes for HIIT and MICT, there were no significant differences on central systolic blood pressure (cSBP) (SMD - 0,13 [IC95%, -0,47 a 0,21], p = 0,45, I² = 25%) and central diastolic blood pressure (cDBP) (SMD 0,15 [IC95%, -0,15 a 0,46], p = 0,32, I² = 20%). As regards to the secondary variables, there were no significant differences for peripheral systolic blood pressure (pSBP) (SMD -0,23 [IC95%, -0,52 a 0,06], p = 0,12, I² = 10%), peripheral diastolic blood pressure (pDBP) (SMD -0,02 [IC95%, -0,30 a 0,27], p = 0,91, I² = 11%), PWV (SMD 0,18 [IC95%, -0,12 a 0,48], p = 0,24, I² = 4%), and AIx (SMD -0,27 [IC95%, - 0,75 a 0,21], p = 0,27, I² = 46%). In the experimental study, both HIIT and MICT significantly reduced the PWV (Δ = -0,37 ± 0,2, p <0,001 and Δ = -0,35 ± 0,3, p <0,001, respectively). Moreover, there were significant reductions on cSBP (Δ = -6,6 ± 6,8, p = 0,010), and augmentation pressure (AP) (Δ = -3,0 ± 5,1, p = 0,033) and a tendency to reduction on AIx (Δ = -6,4 ± 14,8, p = 0,092) and Aix@75 (Δ = -7,6 ± 15,1, p = 0,053) after HIIT and on cDBP (Δ = -3,6 ± 5,0, p = 0,026) after MICT. Conclusion: According to the findings from the meta-analysis, IIT and MICT were similar in reducing cBP, pBP, and AS in healthy and chronically diseased individuals.
Therefore, both aerobic trainings can be applied for the improvement of the central hemodynamic parameters as well as for arterial health. Likewise, in the experimental study, both aerobic training protocols reduced the AS in young obese women. Nevertheless, only HIIT was able to reduce the cBP and the pressure amplification. This
suggests that HIIT might be superior to MICT in enhancing the central hemodynamic parameters in this population.
