Resumo:
Este estudo avaliou nas cinco regiões geográficas do Brasil o impacto que o
posicionamento antroposterior da mandíbula e alterações verticais na dimensão
estômio-mento (St-Me) geram sobre leigos, cirurgiões bucomaxilofaciais e
ortodontistas, suas indicações ao tratamento ortodôntico-cirúrgico, bem como, a
prevalência do gênero, idade, grupo racial, estruturas ósseas e os tipos de
intervenções cirúrgicas mais realizadas em seus pacientes com desarmonias
dentofaciais. Material e métodos: Quatro modelos (duas mulheres e dois homens,
leucodermas e melanodermas) foram fotografados em norma frontal e perfil. As
dimensões St-Me (vertical) foram aumentadas e diminuídas em 10 e 20%, e o
ângulo da convexidade facial (anteroposterior) aumentados e diminuídos em 4 e
8º, totalizando 15 imagens manipuladas para cada modelo (3 originais e 12
alteradas). As imagens foram avaliadas em escala visual analógica por meio de
questionários e foi aplicado o teste de Friedman (com intervalo de confiança de
95%) e o teste de Qui-quadrado. Resultados: De uma forma geral, as imagens

originais, frontais e perfil, foram mais atrativas, porém sem diferença estatística
para as alterações em 10%. No sentido anteroposterior, as imagens originais
foram significativamente mais atrativas e as alterações em 8º as menos estéticas.
Os cirurgiões bucomaxilofaciais e ortodontistas foram mais exigentes que os
leigos, e aqueles com menor grau de escolaridade não conseguiram perceber
alterações faciais verticais mais sutis no perfil. Os melanodermas, depois das
imagens originais, foram significativamente mais estéticos quando apresentaram
verticalmente o terço inferior da face levemente mais aumentado (10%). O oposto
foi observado nos indivíduos leucodermas. Em relação ao gênero, não foi
observada diferença entre eles, as grandes alterações (20% e 8º) foram sempre
menos atrativas, comparadas às pequenas alterações. E entre as regiões
geográficas, houve um consenso em todas elas em relação à atratividade no
ângulo da convexidade facial, as grandes discrepâncias esqueléticas foram
sempre consideradas menos estéticas. Apenas nas alterações verticais,
observou-se diferença entre as regiões Norte e Sul. Quanto às indicações
cirúrgicas, de uma forma geral, os leigos foram mais tolerantes às mudanças na
face. Entretanto, os cirurgiões tanto indicaram quanto se submeteriam mais à
cirurgias caso as imagens fossem das suas próprias faces. Ambos os gêneros
entre os leucodermas receberam as mesmas indicações às cirurgias, quando o
terço inferior da face esteve aumentado (20%) e projetado (8º). Já os
melanodermas diferiram quanto à discrepância sagital. As mulheres receberam
mais indicação à cirurgia quando simulou-se sagitalmente a Classe III esquelética,
já os homens a Classe II. Epidemiologicamente, dentre os profissionais
pesquisados, o gênero feminino foi o mais prevalente (73%) quanto a submissões
à cirurgia em todas as regiões do Brasil, com faixa etária entre 20 e 30 anos

(71%) e leucodermas (75%). A estrutura óssea com mais indicações foi “ambas”
(maxila e mandíbula) com 53,76%, e o mento isolado recebeu as menores
indicações (2,51%). Em relação à maxila e mandíbula separadamente, observou
se diferença entre as regiões geográficas. Nas regiões Norte, Nordeste e Sul a
segunda mais prevalente foi a mandíbula, já no Centro-Oeste e Sudeste foi a
maxila. Em todo o Brasil 7% dos profissionais relataram não realizar tratamento
ortodôntico-cirúrgico, estando a maioria deles concentrados na região Norte
(57%). Das intervenções cirúrgicas de um modo geral, o avanço maxilar foi o mais
prevalente, seguido numa variação muito pequena do recuo mandibular (23,67%
e 23,33%, respectivamente) em todas as regiões. Entretanto, analisando por
região, no Norte, Centro-Oeste e Sul o avanço maxilar foi o mais realizado
(23,33%, 40% e 31,67%, respectivamente), no Nordeste foi o avanço mandibular
(28%) e no Sudeste, igualmente, intervenções em ambas estruturas ósseas
(maxila e mandíbula) foi mais prevalente (25%). Conclusão: A dimensão St-Me e
o ângulo da convexidade facial exerceram forte influência sobre a estética facial e
a percepção para a necessidade de cirurgia ortognática em todas as regiões do
Brasil. Cirurgiões bucomaxilofaciais, ortodontistas e leigos apresentaram
percepções e preferências semelhantes quanto à atratividade, porém os
profissionais foram mais exigentes, e o grau de escolaridade certamente foi fator
relevante para a percepção estética. A maioria dos pacientes atendidos pelos
profissionais avaliados nesta pesquisa com desarmonias dentofaciais foram
mulheres, jovens, leucodermas que apresentam alterações esqueléticas em
ambos os maxilares com indicações e submissões à cirurgias combinadas,
sobretudo para avanço maxilar e recuo mandibular.
Abstract:
This study evaluated, in Brazil’s different geographic regions, the impressions
about mandible sagittal position and Stomium-Mentum (St-Me) dimension
changes on laypersons, bucomaxilofacial surgeons and orthodontists, orthodontic
surgical treatment indications, as well as the prevalence of gender, age, racial
group and types of surgical interventions most referred and performed in patients
with dentofacial disharmonies. Material and methods: Four models (two women
and two men, leucoderma and melanoderma) had frontal and lateral photographs
taken. The St-Me dimensions were increased and diminished in 10 and 20%, and
facial convexity angle increased and reduced in 4 and 8°, totalizing 15
manipulated images for each model (3 originals and 12 modified). The images
were assessed with a Visual Analogue Scale questionnarie, and Friedman test
(95% confidence interval) and qui-square test were applied to the data. Results: In
general, the original images, frontal and profile, were more attractive than the
modified ones, although no statistical significance was shown within those with 0%

change in St-Me dimension. Considering anteroposterior alterations, the original
images were significantly more attractive and those with 8º of modification were
evaluated as the least attractive. Bucomaxilofacial surgeons and orthodontists
were more demanding than laypersons towards modifying the face, and the ones
with lower level of schooling were not able to notice subtle profile vertical facial
alterations (10%). Melanoderma individuals, compared with the original images,
were more aesthetic when presented with a slight increase in lower anterior facial
height. The opposite was observed in leucoderma subjects. Regarding gender, no
difference was observed between them, and significant alterations (20% and 8º)
were, at all times, less attractive when compared to small ones. There was an
agreement between geographic regions concerning facial convexity angle, with
skeletal discrepancies being considered less aesthetic at all times. Diference was
only observed in vertical alterations assessment between North and South
regions. Considering surgical referrals, overall, laypersons were more tolerant
towards facial changes. However, surgeons referred surgical procedures and
would also undergo surgery themselves more often in case the images shown
were from their own face. Both genders, within the leucoderms, received the same
surgical referrals when there was an increase in lower anterior facial height and
mandibular protrusion. Nonetheless, melanoderma individuals received different
indications concerning sagittal discrepancy. Women earned more surgical referrals
when a skeletal Class III was simulated, and men when Class II was presented.
Epidemiologically, female gender had more indications and surgical references in
all regions of Brazil (73%), especially leucoderma ones (75%), within age range of
20 to 30 years (71%). The bone structure with more surgical referrals was “both”
(maxilla and mandible) with 53,76%. Isolated mentum surgery received the least

indications (2,51%). Concerning maxilla and mandible separately, a difference was
observed between the geographic regions. In the North, Northeast and
South regions, the second most prevalent was the mandible, whereas, in the
Midwest and Southeast, it was the maxilla. All over Brazil, 7% of professionals
reported not conducting orthodontic-surgical treatment, with most of these
professionals concentrated in the North region (57%). Taking into account the
surgical interventions in general, maxillary advancement was the most prevalent in
all regions, followed by mandibular setback with a slight variation (23,67% and
23,33% respectively). However, analyzing by region, in the North, Midwest and
South, maxillary advancement was appointed as the most performed procedure
(23,33%, 40% and 31,67%), in the Northeast, it was mandibular advancement
(28%) and in the Southeast, both interventions were equally prevalent (25%).
Conclusion: The St-Me dimension and the facial convexity angle wielded a strong
influence over facial aesthetics and the perception of the need for orthognatic
surgery in all regions of Brazil. Surgeons, orthodontists and laypersons exhibited
similar opinions and preferences regarding attractiveness. Nevertheless,
professionals were more demanding and the level of schooling was a indeed a
relevant factor to aesthetic perception. Most of the patients with dentofacial
disharmonies are women, young, leucoderma, with skeletal alterations in both
maxillary bones, submitted or with indication of combined surgery, predominantly
maxillary advancement and mandibular setback.
