Resumo:
A coparentalidade e auto-eficácia parental têm sido apontados como preditores à saúde dos
cuidadores. Esta Tese de doutorado investigou as relações coparentais e a auto-eficácia de
cuidadores primários de crianças com Paralisia Cerebral (PC). Sua estrutura é formada por
quatro pesquisas em forma de artigos científicos, sendo um estudo de Revisão Sistemática de
Literatura (RSL), e três empíricas. No primeiro, foi desenvolvida uma RSL com o objetivo de
descrever os instrumentos existentes de coparentalidade, assim como traçar o perfil dos
contextos familiares mais investigados pelos mesmos. Os resultados mostraram que as
pesquisas ora desenvolvidas, investigaram a coparentalidade exclusivamente através de uma
abordagem quantitativa, e que os instrumentos disponíveis estavam estruturados em forma de
escalas. Foi possível através deste estudo eleger o instrumento de coparentalidade viável a ser
utilizada nas pesquisas empíricas que seriam posteriormente desenvolvidas. Quanto aos
contextos investigados, foi possível perceber que as famílias nucleares foram os contextos mais
pesquisados. Estes dados deram base para fomentar os objetivos a serem contemplados nos
demais estudos. O segundo e terceiro estudos contaram com a participação de 84 cuidadores
principais de crianças com PC atendidos em um Centro de Referência em desenvolvimento
infantil de Belém-Pará-Brasil. A coleta de dados ocorreu na sala de espera do Centro nos dias
em que as crianças possuíam atendimentos agendados. Os dados foram analisados através da
Técnica de Análise de Correspondência. O objetivo do segundo estudo foi descrever e analisar
as relações coparentais dos cuidadores a partir de suas características sociodemográficas,
observando possíveis correlações entre as variáveis, sendo utilizadas três medidas: Coparenting
Questionnaire (CQ), o Inventário Sociodemográfico (ISD) e o Sistema de Classificação da
Função Motora Grossa (GMFCS). A análise dos dados mostrou cuidadores com baixo nível de
coparentalidade, associado ao gênero e idade das crianças, ao fato destas não frequentarem a
escola, e níveis mais elevados de comprometimento motor. Da mesma forma, constatou-se que
o estado civil e a religião dos cuidadores, a renda e a estrutura familiar, influenciaram suas
relações coparentais. Através da utilização dos instrumentos supracitados, acrescidos de uma
Escala de Auto-eficácia, o terceiro estudo foi desenvolvido com o objetivo de descrever e
analisar, além das relações coparentais, o nível de auto-eficácia parental dos participantes,
mostrando os comportamentos nos quais os cuidadores se sentem mais auto-eficazes. Os dados
mostraram além do baixo nível de coparentalidade, baixo nível de auto-eficácia. Quanto aos
aspectos para os quais se mostraram auto-eficazes, os relacionados à rotina de cuidados aos
filhos foram os principais. Dentre os que se mostraram menos auto-eficazes estiveram aos
aspectos clínicos infantis, como a ocorrência de convulsões. Não foram encontradas
associações estatisticamente significativas entre os escores gerais de coparentalidade e a autoeficácia.
O quarto estudo consiste em uma pesquisa de natureza mista quanti-qualitativa,
constituida por duas fases, sendo que na primeira participaram 84 cuidadores e na segunda,
nove díades coparentais. A coleta de dados deu-se através da aplicação do CQ na primeira fase,
e de entrevistas semiabertas realizadas na residência dos cuidadores, objetivando-se analisar a
percepção dos mesmos sobre suas relações coparentais, sendo suas falas analisadas a partir da
análise de conteúdo. A técnica de Análise dos Componentes Principais foi aplicada aos dados
proveniente do CQ. Através de três níveis propostos de coparentalidade (positiva, média e
negativa), percebeu-se que duas díades apresentaram coparentalidade positiva, cujo fator
principal foi a alta cooperação na divisão de cuidados aos filhos com PC. Cinco díades foram
classificadas com média coparentalidade, sendo observada a presença de cooperação e conflitos
nas díades. Duas díades apresentaram coparentalidade negativa, associada à existência de
conflito coparental e conjugal, e total insatisfação materna. Os dados mostraram a carência de
estudos na área (estudo 1), percebeu-se que contexto de vulnerabilidade socioeconômica e a
baixa qualidade na relação conjugal está associada ao baixo nível de coparentalidade dos
cuidadores (estudo 2 e 4), da mesma forma que se percebeu cuidadores com baixa auto-eficácia
parental quando deparados com uma situação extrema como o enfrentamento de uma convulsão
(estudo 3). Ao fim desta Tese, espera-se que os dados aqui mostrados contribuam para o
esclarecimento acerca das relações coparentais, assim como a auto-eficácia parental.
Abstract:
The coparenting and self-efficacy have been pointed out as predictors of caregivers' health.
This PhD thesis aimed to investigate the coparenting relationships and self-efficacy of primary
caregivers of children with cerebral palsy (CP). Its structure is formed by four researches in the
form of scientific articles, being a study of Systematic Review of Literature (RSL) and three
empirical researches. The first RSL study was developed with the purpose of describing the
existing instruments of coparenting, as well as to draw the profile of the family contexts most
investigated by them. The results showed that the researches developed, dealt with coparenting
exclusively through a quantitative approach, and that the available instruments were structured
in the form of scales. It was possible through this study to elect the viable co-parenting
instrument to be used in empirical research that would later be developed. As to the contexts
investigated, it was possible to perceive that nuclear families were the most investigated
contexts. These data provided the basis for furthering the objectives to be covered in the other
surveys. The second and third studies were the first phase of empirical research, with the
participation of 84 primary caregivers of children with CP attended at a child development
Reference Center in Belém-Pará-Brazil. Data collection took place in the Center's waiting room
on days when the children had scheduled appointments. The objective of the second study was
to describe and analyze the coparentary relationships of caregivers based on their
sociodemographic characteristics, observing possible correlations between variables, using
three measures: Coparenting Questionnaire (CQ), Sociodemographic Inventory (ISD) and
Gross Motor Function Classification System (GMFCS). Data analysis showed caregivers with
low level of co-parenting, associated with the gender and age of the children, the fact that they
did not attend school, and higher levels of motor impairment. In the same way, it was verified
that the civil status and the religion of the caregivers, the income and the family structure,
influenced their coparenting relationships. Through the use of the aforementioned instruments,
plus a self-efficacy scale, the third study was developed with the purpose of describing and
analyzing, in addition to the coparenting relationships, the level of parental self-efficacy of the
participants, showing which care behaviors feel more self-efficacious. The data showed that the
caregivers, in addition to the low level of coparenting, also, for the most part, demonstrated a
low level of self-efficacy. As for the aspects for which they were self-efficacious, those related
to routine childcare were the main ones. Among those who showed less self-efficacy were the
clinical aspects of children, such as the occurrence of seizures. No statistically significant
associations were found between general coparenting scores and self-efficacy. The fourth study
consists of a quanti-qualitative mixed research, consisting of two phases, in which the first one
involved 84 caregivers and in the second, nine coparental dyads. Data were collected through
the application of CQ in the first phase and semi-open interviews conducted in the caregivers'
homes, with the objective of analyzing their perception about their coparenting relationships,
and their speeches were analyzed through content analysis. The Principal Components Analysis
technique was applied to data from CQ, which were answered only by the mothers during the
first collection phase. Through three proposed levels of coparenting (positive, average and
negative), it was observed that two dyads presented positive coparenting, whose main factor
was the high cooperation in the division of care for children with CP and the high quality of
their marital relationships. Five dyads were classified with average coparenting. In this, in
addition to cooperation observed in the dyads, conflicts were verified. Two dyads presented
negative coparenting, associated with the existence of coparental and conjugal conflict, and
total maternal dissatisfaction. The data showed the lack of studies in the area (study 1), it was
noticed that the context of socioeconomic vulnerability and the low quality of the conjugal
relationship is associated to the low level of coparenting of the caregivers (study 2 and 4), in
the same way that Perceived caregivers with low perceptions about their parental self-efficacy
when faced with an extreme situation such as coping with a seizure (study 3). At the end of this
thesis, it is expected that the data presented here contribute to the clarification about the
coparenting relationships as well as the parental self-efficacy.
