DISCUSSÃO DOS DESAFIOS, LIMITES E POTÊNCIAS DAS POLÍTICAS DE CONTROLE DA RAIVA SOB A PERSPECTIVA DA ABORDAGEM ONE HEALTH

Resumo:
As zoonoses são, segundo a Organização Mundial de Saúde, doenças que circulam entre animais vertebrados e humanos, representam riscos à saúde pública, e muitas delas são negligenciadas, ou seja, não são priorizadas pelos sistemas de saúde nacionais e internacionais. A raiva enquanto zoonose, é considerada uma doença negligenciada em alguns países do mundo, causando morte de milhares de pessoas ao ano. Em algumas regiões onde a raiva humana está controlada, causando poucas mortes anuais, a doença ainda está presente circulando entre animais, sendo um risco potencial para os humanos. O Brasil é um dos países que conseguiram controlar a raiva em humanos através de um programa de controle da raiva urbana, com vacinação de cães e gatos e imunização pós-exposição ao vírus rábico. Contudo a circulação viral ainda é elevada, tanto em seu ciclo rural, causando a morte de muitos animais de produção anualmente, como no ciclo selvático, com presença do vírus rábico em morcegos hematófagos e não-hematófagos, raposas, saguis, entre outros. Essa realidade causa perdas de milhões de reais para a pecuária brasileira e ainda impacta negativamente na saúde pública, com elevados gastos com vacinas preventivas. Os programas brasileiros de controle da raiva apresentam, segundo avaliação dos mesmos, dados que podem indicar deficiência na vigilância da circulação viral. O programa de controle da raiva em humanos é executado pelos municípios e o da raiva em herbívoros pelos estados, havendo pouca comunicação entre os dois, inclusive com dados divergentes em relatórios anuais. No Alto Oeste Potiguar, mais especificamente nos municípios que compõe a 6ª região de saúde, ocorreu em 2015 um surto de raiva animal. Nesse contexto, discutiremos no presente estudo as ações dos programas na 6ª região de saúde do estado do Rio Grande do Norte, tendo como princípio norteador a abordagem One Health, que vem sendo instituída em vários países para avaliação das políticas de controle de doenças zoonóticas com bons resultados e otimização dos recursos públicos na sua prevenção. Para tal realizou-se uma pesquisa documental nas fichas de notificação de atendimento antirrábico humano, do período de 2010 a 2015, da VI URSAP, dos municípios de Encanto, Frutuoso Gomes, Pau dos Ferros, Rafael Fernandes, Riacho de Santana e Serrinha dos Pintos. Entrevistas semiestruturadas com alguns gestores e grupos focais com ACE e ACS. Os resultados mostraram que os atendimentos profiláticos desses 6 municípios foram o dobro da média nacional, as pessoas que mais sofreram agravos foram os meninos entre 2 e 9 anos de idade, brancos, com nível de escolaridade fundamental. A maioria dos agravos foi causado pela mordedura de cães e o tratamento mais estabelecido foi a vacinação mais observação do agressor, as condutas de tratamento nem sempre foram condizentes com o preconizado pelo Ministério da Saúde, havendo risco para os pacientes. Os gestores municipais de saúde demonstraram pouco conhecimento das zoonoses, e dos impactos causados por elas, a vigilância epidemiológica inexiste em alguns municípios. Os ACE e os ACS têm dúvidas quanto a transmissão da raiva e as orientações de profilaxia. A cobertura vacinal canina na região ainda apresenta falhas, não há ações educativas, nem de vigilância passiva vem sendo executada de forma contínua nos municípios. Diante de todos esses fatores, conclui-se que a 6ª região de saúde é uma área de risco para o contágio de humanos com a raiva, por insuficiência na execução de ações previstas no PNPR.

Abstract:
Zoonoses are, according to the World Health Organization, diseases that circulate between vertebrate animals and humans, pose a risk to public health, and many of them are neglected, i.e., they are prioritized by the national health systems and international. The rabies as a zoonosis, it is considered a neglected disease in some countries of the world, causing the death of thousands of people per year. In some regions where human rabies is controlled, causing few deaths annually, the disease is still present circulating among animals, being a potential risk for humans. Brazil is one of the countries that have managed to control rabies in humans through a program of urban rabies control, with vaccination of dogs and cats and immunization after exposure to the rabies virus. However the viral circulation is still high, both in its rural cycle, causing the death of many production animals annually, such as the savage cycle, with the presence of the rabies virus in vampire bats and non-Hematophagous, foxes, marmosets, among others. This fact causes loss of millions of dollars for the Brazilian livestock and still impacts negatively on public health, with high costs with vaccinations. The Brazilian programs for rabies control feature, according to the evaluation of the same data, which may indicate a deficiency in surveillance of viral circulation. The program for the control of rabies in humans is run by municipalities and the rabies in herbivores by united, there is little communication between the two, including with diverging data in annual reports. In the Potiguar High West, more specifically in the municipalities that compose the 6th region of health, occurred in 2015, an outbreak of animal rabies. In this context, we discuss in the present study, the actions of the programs in the 6th region of health of the state of Rio Grande do Norte, having as a guiding principle the Approach One Health, which has been established in several countries for evaluation of policies to control of zoonotic diseases with good results and optimization of public resources in its prevention. This was a documentary research on connectors for notification of antirrábico attendant human, the period from 2010 to 2015, the 6th region of health, the municipalities of Encanto, Frutuoso Gomes, Pau dos Ferros, Rafael Fernandes, Riacho de Santana and Serrinha dos Pintos. Structured interviews with some managers and focus groups with community health workers and community agents of endemics. The results showed that the prophylactic handles these 6 municipalities were twice the national average, people who have suffered most injuries were boys between 2 and 9 years of age, white, with level of schooling. The majority of injuries was caused by the bite of dogs and the treatment more established was the vaccination more observation of the aggressor, the ducts of treatment were not always consistent with the recommended by the Ministry of Health, there is a risk for patients. The municipal health administrators have demonstrated little knowledge of zoonoses, and the impacts caused by them, the epidemiological surveillance there in some municipalities. The community agents of endemics and community health agents have doubts about the transmission of rabies and the guidelines for prophylaxis. The canine vaccination coverage in the region still fails, there is educational actions, nor passive surveillance is being performed in a continuous manner in the municipalities. In the face of all these factors, it is concluded that the 6th region of health is an area of risk for infection of humans with rabies, for insufficiency in the execution of actions provided for in the National Program of Prophylaxis of Rabies.
